IMPORTANT REMINDERS:
1. Laboratory requests will be issued after qualifying interviews.
2. Have tests done at any DOH-accredited lab or hospital (e.g., MedicLab, Medicus, MedicWorld, MedicPro, RHUs, hospitals).
3. 𝐔𝐩𝐥𝐨𝐚𝐝 𝐀𝐋𝐋 𝐥𝐚𝐛𝐨𝐫𝐚𝐭𝐨𝐫𝐲 𝐫𝐞𝐬𝐮𝐥𝐭𝐬 𝐢𝐧 𝐭𝐡𝐞 𝐒𝐈𝐌𝐒 𝐩𝐨𝐫𝐭𝐚𝐥 𝐛𝐞𝐟𝐨𝐫𝐞 𝐮𝐧𝐝𝐞𝐫𝐠𝐨𝐢𝐧𝐠 𝐭𝐡𝐞 𝐦𝐞𝐝𝐢𝐜𝐚𝐥 𝐞𝐱𝐚𝐦𝐢𝐧𝐚𝐭𝐢𝐨𝐧

6. Minors must be accompanied by a parent/guardian.
Students must follow their assigned schedule. Those who miss their designated date will have to wait until all other colleges are finished before they can be accommodated. Strict adherence to schedule will be implemented.
